The following forms and waivers can be found on this page
1. WDD LIABILITY WAIVER, PRIVACY POLICY, AND TERMS
ALL clients must agree to the safety and liability waiver, privacy policy, and terms and conditions when becoming a client with WDD.
2. WDD COVID-19 WAIVER
ALL clients must sign the COVID-19 waiver when becoming a client with WDD.
3. WDD NEW CLIENT INTAKE FORM
ALL clients must fill out the new client intake form and upload their vet records when becoming a client with WDD.
4. WDD SECOND DOG INTAKE FORM
Optional intake form. Only fill this out if you're bringing a SECOND dog from your household to WDD.
5. WDD THIRD DOG INTAKE FORM
Optional intake form. Only fill this out if you're bringing a THIRD dog from your household to WDD.
6. WDD GROOMING WAIVER
Ethical and humane grooming waiver. All clients who are booking a grooming must complete this form for EVERY session.
AS OF 01/17/2023 WDD CEASES OPERATIONS AT 1 TANNERY CIRCLE
On 01/13/2023, WDD stood before a NYS Supreme Court Judge petitioning to extend our deadline to file an Article 78 Proceeding and to extend the Order of Remedy put in place by the Village of Dryden.
The Judge ruled to allow for the Article 78 Proceeding to be extended (yay!) but also ruled to not extend the Order to Remedy (boo!) effectively ceasing operations of WDD at 1 Tannery Circle.
However, we are not entirely closed! We have converted to conducting home visits in lieu of daycare!
